Privacy Policy 

The policy complies with requirements under the 1998 privacy act. Data will be collected to ensure all medical history and information is up to date to mange health and ensure quality treatment is provided, use for financial claims or audits may be necessary (RACGP, 2022). Data is collected via registration forms, during the course of providing services (during consults, emails, recorded phone calls). Via transfer of information from other health practitoners (general practitoners, psychologists, lab results) this transfer of information is only possible through signed consent. Sharing of personal information to other healthcare providers, if it is required by law or in court, assitance in missing person or patient/public health or safety (RACGP, 2022). Personal information is stored via electronic format in password encripted protected databases and kept for 7 years or until the patient is 25 years before medical data is destroyed if no longer needed by a commercial service provider. Destruction of any medical doccuments will also be doccumented, confidentiality agreements signed by staff. The practice acknowledges patients rights to acess medical records, which is requried in writing and provided in 30 days including fees if applicable. Privacy related complaints are taken seriously, written and documented concerns will be attempted and resolved in accordance with resolution procedure (RACGP, 2022).
